Please copy or print the form, and complete it (modify/add to the areas relating to reservation details if necessary, but do not change the conditions or sense of the payment authorisation, otherwise it will not be accepted). Then send it via post or fax to HPS Hotel Reservations.
Once HPS has confirmed the rooms with the hotel we will forward your accommodation voucher/s
by fax /post or e-mail.
This authorisation is particularly important if you are using a third party card (e.g. a company card, a relative's or a friend's card)

25% where the cancellation/reduction is made more than 42 days prior to departure
45% where the cancellation/reduction is made more 41 and 15 days prior to departure
